At Align Occupational Therapy, we provide occupational therapy driving assessment and driver rehabilitation programs to individuals with disabilities and medical conditions.
The following documents and steps are required to be provided prior to an OT driving assessment taking place:
Doctor’s Referral:
We require a doctor’s referral for an OT driver assessment, noting your medical history, medications, and any concerns your doctor has about your driving.
Vision Assessment:
If you have had visual changes associated with your medical condition or if you have a neurological condition, we require an optometrist or ophthalmologist report with your referral. If you have not had changes to your vision a basic test with your GP is usually sufficient.
Licence:
You must have a current and valid driver’s licence . This may include a Class-C (car) licence or a learner’s permit. Cancelled or suspended licences are not valid, therefore an assessment cannot be completed.
Transport Authority Medical Forms:
If you live in QLD, the Department of Transport & Main Roads may require you to fill out the ‘Medical Certificate for Motor Vehicle Driver’ form. The transport department will advise you if you need to have the form completed when you notify them of your medical condition/disability. You can obtain this form in person from a service centre, or online at www.support.transport.qld.gov.au/qt/formsdat.nsf/forms/QF3712/$file/F3712_CFD.pdf. You must take this form to your GP, who may write on the form that you require an OT Driving Assessment.
